Financial Policy

For patients that have Dental Insurance, we can electronically submit to your insurance company and they will reimburse you. Dental Insurance plans can vary. The procedures and extent of coverage are determined by the employer and the insurance carrier. Dental plans may not take into account your dental needs. The treatment Dr. Fok or Dr. Chau recommends is based on the disease present and may not always be covered by your dental plan.

At the consultation Dr. Fok or Dr. Chau will carefully examine your mouth to diagnose your condition. You will have the opportunity to discuss your problem and ask questions. Treatment options will be presented and an estimate or predetermination will be given to you. Our staff will be pleased to assist you with your insurance forms. A predetermination form can be electronically submitted PRIOR to your treatment so you will know the extent of your anticipated reimbursement.   

You are responsible for the payment of fees (we are a non-assignment office). Payment is due at the time of treatment unless prior arrangements have been made. Fees can be paid by Cash, Visa, Mastercard or Debit.
